Registered Nurse (RN) – Home Health

Territory: Bloomington | Full-Time | Field-Based

-----------------------------------------------------------------------------------What You’ll Do

As a Registered Nurse in Home Health, you will:

• Deliver skilled nursing care to patients in their homes

• Complete comprehensive patient assessments and develop individualized plans of care

• Serve as the primary nurse for your caseload—assessing, planning, coordinating, implementing, and evaluating care

• Perform initial evaluations, routine visits, and re-evaluations based on patient needs

• Monitor and document patient progress, changes in condition, and outcomes

• Communicate significant findings and care updates with physicians, clinical leadership, and the interdisciplinary team

• Revise care plans as patient needs evolve

• Ensure compliance with HIPAA and all regulatory standards

Required:

• Current, unrestricted RN license in the state

• At least 1 year of clinical nursing experience

• Ability and willingness to travel within the Bloomington territory (approximately 50%)

• Valid driver’s license, reliable vehicle, and current auto insurance

Preferred:

• Associate’s or Bachelor’s Degree in Nursing

• Previous Home Health or Hospice experience

• Experience with HomeCare HomeBase
